Acid Reflux and Heartburn on a GLP-1
Heartburn, burping, and reflux are commonly reported on GLP-1 and tirzepatide medications. They are annoying, and sometimes they are a sign to talk to your prescriber. In Zepbound's weight-loss trials, 4% to 5% of adults reported reflux (GERD), compared with 2% on placebo.
Why It Happens
These medications slow how fast your stomach empties. Food stays in the stomach longer, which can push acid upward, especially after larger meals or when you lie down soon after eating.
Practical Strategies
- Eat smaller meals and stop before you feel full. Overfilling a slow-emptying stomach is a common trigger.
- Avoid lying down for two to three hours after eating.
- Notice your personal triggers. Many people find fried food, tomato, citrus, alcohol, caffeine, and carbonated drinks make it worse.
- Raise the head of your bed a few inches if symptoms hit at night.
- Talk with your prescriber or pharmacist before starting an antacid or acid reducer, especially if you take other medications.

Questions to Ask Your Clinician
Bring these to your next appointment. Your prescriber is your best resource for decisions specific to your situation.
- “Is my reflux related to my medication or dose?”
- “Which acid reducer, if any, is safe alongside my other medications?”
- “Should I pause a dose increase while this is happening?”
- “Which symptoms mean I should be evaluated in person?”
