Dizziness and Lightheadedness on a GLP-1
Feeling dizzy or lightheaded can happen on GLP-1 and tirzepatide medications. In Zepbound's weight-loss trials, 4% to 5% of adults reported dizziness, compared with 2% on placebo.
Why It Happens
Eating and drinking less, plus vomiting or diarrhea, can leave you dehydrated or with low blood pressure. The Zepbound label notes that low blood pressure was reported more often with the medication (1.6%) than with placebo (0.1%), and that it also occurred along with stomach and gut side effects and dehydration. If you have diabetes, low blood sugar is another possible cause.
Practical Strategies
- Stand up slowly, especially in the morning and after sitting for a long time.
- Keep drinking fluids through the day, and add electrolytes on days with vomiting, diarrhea, heat, or exercise.
- Do not skip meals completely. Small, regular meals help.
- If you have diabetes or take diabetes medicine, ask your prescriber what blood sugar checks make sense for you.
- If you feel faint, sit or lie down right away.
- Be careful with driving and stairs until you know how you feel.

Bring these to your next appointment. Your prescriber is your best resource for decisions specific to your situation.
- “Could my dizziness be dehydration, low blood pressure, or low blood sugar?”
- “Should my blood pressure or blood sugar medicines be adjusted as I lose weight?”
- “Should I have my kidney function or electrolytes checked?”
- “What readings or symptoms should make me call you or go to urgent care?”
